We have new Facility Request Forms!!
Recently we've changed the forms that must be submitted when requesting the use of facilities. Please be sure you are using the current and appropriate request form for your desired facility request. When your request is approved, your original copy of the request form will be returned to you via inner-office mail. Please contact the Office of Support Services if you cancel your event. This will help us provide professional and prompt service for other events and programs.
